Job Title: Direct Support Professionals
Reports To: Facility Manager
Job Description: Provides personalized care and supervision of residents assigned to him/her. Serves as a universal worker performing different tasks to promote consumers well being and satisfaction through support in daily living activities. Primarily involved in creating a caring and supportive environment for all residents and promote individual
satisfaction. Attends staff meetings and training as directed.
Qualifications:
- Six (6) months experience working for people with developmental disabilities.
- 18 years of age or above.
Possess a high school diploma or pass a general educational development
test (GED). Good character and interpersonal skills. - Must possess a valid drivers license.
- Proof of good health through a physical exam and TB clearance in the past six months.
- Fingerprint Clearance.
- First Aid and CPR certificates.
Duties/Responsibilities:
- Assist residents in the activities of daily living such as bathing, dressing, grooming, oral hygiene and mobility.
- Physical ability to perform the duties, including the ability to assist with mobility and consumer transfers as needed.
- Communicate effectively with the residents during the day. To include communicating consumers special needs to health professionals, etc.
- Follow the facility schedule of duties for consumer assistance, as well as the individual serviced plan and individual program plan for each consumer.
- Function as a team member by assisting co-workers as the need arises.
- Dispense and safeguard medications.
- Insure that all medications are administered following the facilities medication policies and procedures.
- Prepare nutritious meals and snacks based on posted weekly menus and make appropriate substitutions per consumer choice.
- Must possess the capability to read, understand and carry out per each residents Individual Service Plan.
- Help facilitate and/or conduct planned activities with the residents.
- Help monitor resident behavior, food intake and functional capabilities.
- Help implement the individual care plan for each resident.
- Report resident health status change to immediate supervisor.
- Act immediately on all consumer crisis, following protocol and basic first aid training.
- Document consumer status change, including but not limited to, physical change, reaction to medication, psychosocial status change, and/or behavioral changes.
- Assist maintenance person in keeping the facility clean and free from odor at all times.
- Complete laundry, sort and orderly put them in their respective places if appropriate.
- Follow universal precautions while on duty especially when providing care to residents.
- Read and understand the personnel policies contained in the Employee’s Manual.
- Adhere to the guidelines in the employee handbook including dress code and conduct.
- Perform other tasks that maybe assigned by immediate supervisor.
